Primary Care

The clinical team consists of physicians, physician assistants, nurse practitioners, nurses, medical assistants, medical secretaries, dietitians, diabetes educators, and behavioral health consultants.  The NHBP medical facilities provide a full spectrum of primary care services included, but not limited to:

 

  • Wellness exams throughout one’s lifespan
  • Primary care for all ages: newborn to elder
  • Medical management and treatment of chronic conditions such as diabetes, asthma, chronic obstructive pulmonary disease (COPD), high blood pressure, etc.
  • Medication review and management
  • Medical management and treatment of mental health problems such as depression , anxiety and ADHD
  • Contraceptive management and well woman care
  • Immunizations, including flu and COVID vaccines
  • On site lab for many common tests including cholesterol, blood sugar, hemoglobin A1C, electrolytes, kidney function, strep throat, COVID-19, HIV, etc.
  • Blood draw site for laboratory testing
  • Department of Transportation (DOT) physicals
  • Sports physicals
  • Referrals for specialty care and preventative health screenings
  • Primary care procedures such as skin biopsies, wart treatment, joint injections, drainage of abscesses, etc.

Same Day Care

The NHBP medical team provides same day visits for the treatment of acute illness and injuries such as:

  • Sore throats and flu-like symptoms
  • Ear infections
  • Rashes
  • Simple lacerations and abrasions
  • Minor strains and sprains
  • Minor burns
  • Urinary tract infections
  • Sexually transmitted diseases

    Additional Services

    Medication Assisted Treatment for Substance Use Disorders

    Our medical facilities provide medication assisted treatment for both alcohol use disorder and opioid use disorder.  The individuals receiving medication assisted treatment have access to a behavioral health care manager and the other substance use disorder services offered through our Behavioral Health Department including peer support, Wellbriety meetings and individual counseling.

    Integrated Care

    Our medical facilities utilize an integrated care approach to provide more comprehensive care to people with complex health problems.  The care team includes a primary care provider and can include a nurse care manager, behavioral health consultant, dietician, diabetes educator, health educator, or community health representative depending on the needs of the individual.  The care team works together with the individual to achieve their health goals and improve their overall health and wellness.

    Dietitian

    Our dietitian offers one-on-one nutrition therapy both in person and by telehealth.  Nutrition therapy can be beneficial for individuals with many medical concerns including diabetes, hypertension, elevated cholesterol, obesity, and gastrointestinal concerns such as GERD (reflux), IBS (Irritable Bowel Syndrome), and chronic constipation.

    Diabetes Self-Management Education and Support

    Our dietitian is also certified in Diabetes Self-Management Education and Support and offers individual diabetes education in person and by telehealth.  This education is designed to help individuals with diabetes learn to manage their diet, medications, and activities in order to achieve optimal diabetes control.  NHBP Medical Facilities also offer Continuous Glucose Monitoring for patients with diabetes along with education and assistance with home Continuous Glucose Monitoring.

    Care Management Services

    Care Management is a comprehensive set of services that help individuals with chronic or complex medical conditions manage their medical care and improve their health and wellness.  Nurse Care Managers are available at the Pine Creek and Grand Rapids facilities to work one-on-one with individuals to help them create and achieve their health goals.  The care managers help individuals navigate the health care system to make sure they are receiving all of the recommended referrals, testing and screening.  Care management is particularly important after a hospitalization or emergency department visit to make sure the individual understands the plan of care and can maintain their safety at home.  The Nurse Care Manager is the main point of contact between the individual and the health care team and provides help with coordinating medical care, medication management, health education, and connecting individuals with social and community support systems.

    Behavioral Health Consultants

    NHBP recognizes the importance of treating and supporting the whole self. Behavioral Health Consultants (BHC) play an important role in this goal by providing same-day access to a masters-level clinician who can connect individuals with the right level of behavioral health services. The Pine Creek and Grand Rapids facilities have Behavioral Health Consultants available to individuals at the time of their medical appointment.  Often the BHC can help address concerns through brief intervention, and if more support is required, they can assist with coordinating this care. In addition to assisting with behavioral health concerns, a BHC can help with a broad range of needs and medical situations, including food access and transportation concerns to significant life changes and chronic illness diagnoses, like obesity and diabetes. Your provider may have a BHC stop in to see you at your next medical visit, and you are also welcome to request to see a BHC at your visits.

    Eligibility for Services

    • All federally recognized Tribal Members and descendants
    • Adopted, foster, step and legal ward children of a federally recognized Tribal Member
    • Mothers of unborn American Indian/Alaskan Native (AI/AN) children
    • Spouses of all federally recognized Tribal Members
    • NHBP employees
    • Residents of Fulton, Athens, East Leroy and Union City are eligible to receive medical services at the Pine Creek Health Facility
    • Team members of FireKeepers Casino Hotel are eligible to receive medical services at the NHBP Heath at the FireKeepers Facility

        Insurance and Charges

        There are no out-of-pocket fees for direct care services provided at the NHBP Health Facilities for American Indians and Alaskan Natives. However, any available commercial insurance, Medicaid, Medicare or grant funds may be billed for services. Services which cannot be performed at the facility (i.e. x-rays, certain lab tests, referrals, etc.) may incur out-of-pocket fees.
